About The Role

An exciting opportunity has arisen to work as a Continuing Heath Care(CHC) Social Worker with the Adult Response Team.  We are seeking a dedicated and experienced (CHC) Social Worker to join our team. This role involves working collaboratively with multidisciplinary teams to support individuals who are eligible for NHS Continuing Healthcare funding. You will ensure that care is delivered effectively, with the client’s well-being at the heart of every decision.


As a Social Worker, you will:

be carrying out the full spectrum of statutory responsibilities under the Care Act, Mental Capacity Act and other related law and policy.  You will be expected to carry out your role in a strengths based way, to a high quality, and always seeking to reflect on and continually improve your practice. You will be fully committed to working in partnership with residents, carers, community- based organisations and other key professionals involved in enabling people to be as independent as they possibly can.

Undertake CHC assessments in line with NHS frameworks and guidelines.

Work collaboratively with health and social care professionals to develop personalized care plans.

Advocate for clients and their families, ensuring their voices are heard throughout the CHC process.

Support decision-making around funding eligibility and care planning.

Ensure compliance with legal and regulatory frameworks, including the Care Act 2014 and Mental Capacity Act 2005.

Maintain accurate and timely records of all interactions and decisions.


You will be a core member of a multi-disciplinary team of social workers and case managers. The team is extremely fast paced and sits at the heart of Council’s corporate customer centre to deliver positive outcomes for residents with adult social care needs. 

 

About You:

To excel in this role, you will need:

A degree in Social Work and registration with Social Work England (or equivalent body).

Proven experience in a health or social care setting, preferably involving CHC.

Strong knowledge of NHS Continuing Healthcare frameworks, the Care Act, and relevant legislation.

Excellent communication, advocacy, and organizational skills.

The ability to work collaboratively as part of a multidisciplinary team.
